Device Registration: Purchasers of radiological equipment must register their device with the New York State Radiation Control Board.

Variance Forms: In addition to the device registration, both the state and city of New York require separate variance request forms for dental and veterinary use. The purchaser should fill out the appropriate form and mail it to the addresses shown.
